Student IDs for Students of Polish Schools Outside of Poland

We wish to inform you that IDs obtained in the last school year can be extended for this year - please fill out the form and attach your current ID. If you wish to obtain a new id, please fill out the form. We ask that any who wish to apply for this, please return your completed forms to your teachers by March 24th.
